Please save this to your desktop and open it in Adobe Acrobat Reader so it will have full functionality. The dates need to be entered in this format DD/MM/YYYY to work properly or you can use the drop-down calendar option. This can then be printed and signed, barcode label added and sent with the sample as usual.
